THE CENTER’S CORE SERVICES

A major goal of the Center is to promote the adoption and spread of the Pathways Model nationwide. To accomplish this goal, the Center will need to develop and implement a diverse range of services and activities.   Proposed core services and functions of the Center include: 

 

 

 

 

 

Research, Implementation, and Evaluation

Conduct benchmarking studies to build an evidence base for community care coordination services.

Estimate the ROI of Community Care Coordination Services through education in hospitalizations, emergency

   room use, and other avoidable costs.

Demonstrate the impact of care coordination services on disparities reduction and health outcomes.

Conduct evaluation studies of different program models.

Publicly report performance data.

 

 

Education & Training

Facilitate virtual Learning Networks of collaborators in diverse communities

Conduct educational briefings to promote the role of  community care coordination in health care reform.

Conduct policy analyses and disseminate reports on community care coordination initiatives around the

  country.   

Convene conferences, seminars and workshops

 

 

Capacity Building & Technical Assistance Services

Help Implement Pathways programs by providing a range of strategic, developmental, and technical support in

   conjunction with education and training.

The Center will provide technical assistance to new programs interested in  obtaining certification

 

 

Product Development

Develop Quality Improvement Tools to facilitate program development and operations.

Develop training materials (i.e., guides, manuals, etc).

Produce publications & disseminate information on research findings and best practices.
